Requesting your transcripts from any of Concordia University’s online programs is easy. Simply, download the transcript request form below and fax or mail it to the Office of the Registrar at 503-280-8661 or 2811 NE Holman St Portland, OR 97211.

Unofficial Transcripts

Unofficial transcripts can be faxed to you at no charge. Students are limited to one copy of their unofficial transcript per semester.

Official Transcripts

Official transcripts can be requested for $5.00 per copy. They can be recieved same-day for $10.00 per copy.
